2.I received a check directly from my insurance company...

Sometimes insurance companies might mail you a check directly to cover testing services provided by Catalyst Diagnostic. These checks should be endorsed by yourself and mailed with your EOB to our mailing address: Catalyst Diagnostic Billing 4942 Dawn Ave, Ste 210 East Lansing, MI 48823

3.Are discounts available for testing services?

Catalyst Diagnostic has programs for eligible patients that require financial assistance or currently do not or are not using insurance for payment. Contact us!

4.Do you accept secondary/supplemental insurance?

We can work with you to obtain the additional information necessary to submit and optimize the benefits that you have. If you have secondary or supplemental insurance information to provide, please give us a call at (517) 885-3556, Menu Option 5

5.Will my insurance cover testing?

Catalyst Diagnostic is a participating provider in many insurance networks. You can reach out to our billing department directly and inquire whether we service your specific insurance plan.

6.What is an Explanation of Benefits (EOB)?

Everytime Catalyst Diagnostic bills your insurance company for testing services ordered by a physician for you or your dependents, the insurer will send you an Explanation of Benefits (EOB). This EOB gives a detailed explanation at what they will pay and an anticipated amount that you might need to financially cover. It is important to note that an EOB is NOT a bill and you will receive a bill directly from Catalyst Diagnostic with your responsibility.

7.Why did I receive this bill?

The bill that you received is for laboratory services that were performed by Catalyst Diagnostic on behalf of your physician. Charges that appear on this invoice reflect testing fees that are separate from any fees paid at your physician office. Items on here might include deductibles, co-payments and items not covered by your insurance. If the billing information provided to Catalyst Diagnostic was inaccurate or missing information we will ask that you provide the additional information to us.
